This Community Health Needs Assessment survey is hosted by FW Medical Center with assistance of the Jefferson County Alliance of Service Councils, Inc., and the Jefferson County Health Department.
Please see the attached informational card. The surveys can be done by using the QRS code, online or by paper copy. We are asking residents to please complete by September 1st.
